Membership is an annual contract. You may cancel at any time with a 30 day written notice. Re-enrollment fee of $250 will be applicable if members cancel then re-enroll at a later date. Family maximum limits available when families sign up together. There are no restrictions on frequency, type or duration of office visits per month.
