In Person visits $80 per person, Telehealth $50 per person; One-time registration Fees $50 for membership plans up to 3-4 members, $100 for 5-6, and $150 for 7 + memberships. Memberships allow the family or individual to have continuous available access to me as their provider. Once established with their first in home visit, many needs won't require a visit and can be discussed via the phone/messaging allowing for more cost savings.
